How to file an incident (minor infraction) report


1. Log in to your CIS-3G and click on Menu > Student Conduct > Record Incident


2. Search the student by typing in their last name and first name and click on the Student Search button.


3. Select the student from the menu below the search box.


4. Click Insert


5. Fill out all of the box displayed in the sample picture below. Also do not forget to check the box next to where it says Student Notified. In the comment box you can say something like: The student was scheduled for TABE testing today at 1PM but he/she failed to show up without a good reason. After you are done filling out the form click on the Save button and the box above will populate.


6. Click on PDF to download the incident report (in a PDF file) and save it to your local drive.  NOTE: Make sure you select Minor Infraction as the type of incident and NOT a level 1 or 2 incident because level 1 and 2 are for removing the student from the program.



6. Print out the incident report, sign it and send it to your supervisor and the PBSO (Center Standards) manager. (See where to sign and below)
